Registering with Practice1 

If you are living within the practice area and wish to register with the Practice, a registration form and questionnaire can be obtained from reception

ADULTS OVER 16
  •  Complete the registration Form and the practice Questionnaire.
The  Registration Form MUST been signed in the bottom left corner of the front page and handed back in at reception. if this is not signed this will cause a delay to your registration . 
  • Both forms can be dropped off at reception or placed in the prescription box outside. 
  • All patients over 16 need to have a new patient medical with the practice nurse which need to be booked at reception or via the telephone. 
  • A copy of repeat prescription will help enable us to prescribe your medications quicker. 
  • Your NHS number is importannt on documents and can speed up teh registration process also. This is a 10-digit number, like 485 777 3456.  This can be found on majority of your documents such as prescriptions, hospital letters, appointments letters. Your previous practice can help you with this information 

Attached to your registration pack you will also receive a behavioural contract which will also need to be signed before acceptance to the practice. This states that we do not tolerate any aggressive abusive behaviours and that you will not act in this way.
