Complete your tear off slip and post it through our letter box on the front door or write the required medication on a piece of paper with your details and post this through. |
By Post |
You can post your prescription request to us and this will be processed within 48 hours of receiving your request. If you enclose a stamped addressed envelope we will send your completed prescription back to you. |
On Line |
If you have a My Health Online account, you can request repeat prescriptions using My Health Online. Further information on My Health Online can be found at the Online Services page. |
Via a Pharmacy | If you are signed up to a preferred pharmacy, contact your pharmacy to order your medication on your behalf. |

YOU MAY BE REQUIRED TO HAVE A MEDICATION REVIEW WITH THE PRACTICE PHARMACIST AT SOME POINT BUT WE WILL ADVISE YOU OF THIS.
Please give as much detail as possible regarding the medication you are ordering including Drug Name, Directions and Quantities. You request will not be processed without these full details causing a delay in your request.
REMEMBER: Pharmacies offer the Common Ailments Service and a Walk-in Centre facility. They can provide confidential advice and issue FREE prescriptions, including emergency hormonal contraception and emergency supplies of medication.
